model casting form

If you are interested in collaborating on a project with me, please fill out the questionnaire below to be considered for casting calls. For castings, I am generally looking for models with a dress size of 0-6 and occasionally 8-10. 

NOTE: I am based in the Winston-Salem / Greensboro area. Filling out this form indicates that you are willing to travel to the area.
